If a person can aim decently, instead of hitting the liver, kidneys, into the diaphragm or whatever, a sucker punch isn't likely to do significant damage. I wouldn't let someone do it I didn't trust. I'm pretty confident about a punch bottoming out just above my navel, without causing damage, but I'm sure it would lead to a bit of pre-cum.
